Graduate Student Forms

Independent Study Form (ANTH 596): this form should be signed by student and advisor, then submitted to DGS for approval, and sent to program coordinator by the 8th day of the semester for Fall and Spring enrollment. Permission will be entered into the system, and student will then register as usual online, adjusting to the correct number of credit hours. Students can apply up to 12 hours of independent study toward the MA or PhD.
Committee Chair Agreement Form: to be completed as part of the paperwork before officially moving from the MA to the PhD. For students entering the program with a prior Anth MA, this form should be completed by the Fall semester after completing the core courses.
Committee Member Agreement Form: to be completed when preparing for preliminary exams. Must have signatures (or emailed agreements) from each committee member before filing the GC Committee Recommendation Form for the preliminary exams.
Petition to Postpone Prelim Exam: preliminary examinations must be completed with one year of completing the MA, or if entering with a prior Anth MA, within a year of completing the core courses. If circumstances necessitate a delay, this form should be submitted to request a postponement.
Language Proficiency Form: the individual committee chair determines which language, the level of knowledge necessary, and the method of demonstrating proficiency. While we do not have a specific deadline, we encourage students to take care of their language training early in their career.
Dissertation Approval Form: to be completed when preparing for the dissertation defense. Must have signatures (or emailed agreements) from each committee member before a defense can be officially scheduled and advertised.
Graduate College (GC) Committee Recommendation Form: This form is used for both preliminary examinations and dissertation defenses. A CV for any committee member outside of UIC must be submitted along with the form. For prelims: a title is not necessary; be sure to complete question 4 in the regulatory issues box (1-3 are not required). For defenses: be sure to answer all four questions in the regulatory issues box, and make sure your name and title appear exactly on this form as they will on the final dissertation. Graduate College Forms: Includes link for online petition for extensions and zero hour requests, as well as information for forms related to admission, registration, leave of absence, and exams and defense. Forms that must be completed in quadruplicate can be obtained from program coordinator, or from GC main office (UH 6th floor).
